Information on Personal Data Processing

 

I hereby acknowledge that the company:

registered office at Příčná 129/3, 77900 Olomouc,
registered in the Commercial Register maintained by the Regional Court in Ostrava, Section C, Insert 89171

(hereinafter also referred to as the “Controller”)

processes my personal data to the extent of: first name, last name, residential address, phone number, email address, for the purpose of processing my order on the e-shop https://www.uniqueideas.cz, fulfilling obligations arising from the purchase agreement concluded between me and the Controller via the Controller's online store, and related services of the Controller,

in accordance with Article 6(1)(b) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (hereinafter referred to as “GDPR”).

I also acknowledge that until the GDPR becomes effective (i.e., until 25 May 2018), the data is provided under Act No. 101/2000 Coll., on the Protection of Personal Data and on the Amendment of Certain Acts, as amended.

I am aware of and consent to the fact that the Controller may provide the personal data I have supplied (to the extent stated above) to other processors for potential further processing of personal data, who process personal data on behalf of the Controller and provide guarantees of appropriate and proper personal data processing and adequate protection of personal data, including protection through appropriate technical or organizational measures against unauthorized or unlawful processing and against accidental loss, destruction or damage.

The personal data provided to the Controller will be processed for the duration of the contractual relationship with the Controller and subsequently for the necessary archiving period in accordance with the applicable legal regulations of the Czech Republic.

I further declare that I am aware of my right to information about the processing of my personal data (in particular, information about the category of processed personal data, purpose, method and duration of processing, and any recipients of the personal data), the right to request access to my personal data from the Controller, their rectification or erasure (especially if the data is incorrect, incomplete or outdated), and the right to lodge a complaint with a supervisory authority. I understand that from 25 May 2018 I also have the right to data portability to a new personal data controller and the right to restriction of processing.

I also declare that the personal data I have provided is true, I have provided it completely freely, knowingly, voluntarily, and I am aware that its provision is necessary for the provision of the above-mentioned service.
